Licensure Applications
The state of Kentucky provides the following types of licenses- Physician, Resident, Institutional Practice, Faculty and Fellowship. To apply for any of these licenses follow the instructions below. It is your responsibility to confirm licensure eligibility prior to completing the application. Licensure fees are non-refundable.
- Once you click on the link below, you will be required to create a login profile by choosing "Don't have a login, click here." on the left side of page.
- Once you have created a login profile, you have initialized your application process which consists of demographic information only.
- Upon completion of the login profile, you will be prompted to "Click Here To Apply For License."
- After completing the online application, you will be prompted for a non-refundable application fee. Payment may be made by credit card or ACH debit.
- Once the payment is complete, you will be directed to your profile screen where you will see a pdf link for application instructions and forms. These forms are required for your application to be considered complete. Please note these forms are fillable for your convenience.
- If you currently hold a KY Training License, please proceed to the next section and follow the instructions provided in that section.

If you currently hold or have ever held a Kentucky Institutional Practice Limited License or Residency Training License and are applying for a Physician License follow the instructions below.
- Once you click on the link below, you must log in with your current or previous Kentucky license number as your username and the last four digits of your social security number as your password. Do not create a new login profile as this will delay the licensing process and require additional payment.
- Choose "If you do not wish to renew your license but upgrade to a Physician License click here." After completing the online application, you will be prompted for a non-refundable application fee. Payment may be made by credit card or ACH debit.
- Once the payment is complete, you will be directed to your profile screen where you will see a pdf link for application instructions and forms. These forms are required for your application to be considered complete. Please note these forms are fillable for your convenience.

If at any time you need additional and/or duplicate application forms, you will need to log into your profile and access the instructions/forms there. This is the only way to obtain these forms.
