Physician Assistant Information
A physician assistant is a person who has graduated from a physician assistant or surgeon assistant program accredited by the Accreditation Review Commission on Education for Physician Assistants and has passed the certifying examination administered by the National Commission on Certification for Physician Assistants.

The following application is intended for new physician assistant applicants.
If you are a physician assistant currently licensed in the state and are changing jobs, your new supervising physician must complete an application for supervision. Board approval must be granted prior to practicing under a new supervising physician. If at any time a supervising physician ceases to employ or supervise you, you must notify the Board, in writing, within three (3) business days.

Renewal/Re-Registration |

Physician Assistants are required to renew their license bi-annually. The Board will send renewal notices by mail approximately 60 days prior to expiration.
