Change of Address

Address changes to your mailing and practice address can be made online.  Please follow the directions below:

Before clicking on the link to change your address, please read the following instructions:

• To log on to the system, enter your KY license/certificate number as the username and the last four digits of your social security number as your password and click the log in button.
• If you are an applicant for licensure/certification and do not have a license, please use the username and password assigned to you by our licensing department.
• Once the screen loads, choose My Account on the left side of the page.
• Choose Addresses under the My Account tab.
• Choose the pencil icon next to the address you wish to update and save your changes. 

       Click here to log on and change your address

• To log on to the system, enter your KY license/certificate number as the username and the last four digits of your social security number as your password and click the log in button.
• Once your profile loads, please click on Update to the right of your name and address information.  This wizard will only allow you to change your email address and phone number.  Please see above instructions to change your practice and mailing address.
• Once you have completed the wizard and made changes, click on Save Changes at the bottom of the screen.
